WHEN WILL I RECEIVE MY ID CARD?

Your ID card, Schedule of Benefits and Certificate of Insurance will be mailed to the address provided on your application within 1-2 business days after issue date.

WHEN WILL MY PREMIUM BE DEDUCTED?

Whether you choose automatic bank account withdrawal or credit card, your initial premium will be deducted between the 1st and 10th of the month your policy is effective.

For example:
  • Issue date: 10/15
  • Effective date: 11/1
  • Premium deducted: 11/1 – 11/10

    WHAT IS MY GROUP NUMBER?

    Your group number is located on your ID card.

    HOW CAN I MAKE SURE THAT MY DENTAL PROVIDER KNOWS WHAT SERVICES ARE COVERED?

    Provide the dental office with both your group ID and group name from your ID card when you make the appointment. Also, make sure to take your ID card and certificate to your first dental appointment.
    Providers can check eligibility and claim status through the fax recall system at 877-223-4693. All services expected to exceed the predetermination amount of $300 should be reviewed by IHC Health Solutions.

    WHAT DOES A DENTAL PROVIDER NEED IN ORDER TO SUBMIT A CLAIM TO IHC HEALTH SOLUTIONS?

  • An IHC Health Solutions claim form – many providers use their own claim form
  • The Social Security number or alternate ID number of the insured
  • The address or electronic payer ID found on the back of the ID card
  •
